Aestiva Transfer Form
Product to Professional Support

This form hereby confirms the transfer of your support program from Product Support to Professional Support and Product Upgrades. Such transfer shall be effective the transfer date ________________. The ending term shall not change.

Customer understands Professional Support is billed at the same hourly rate as Product Support but Professional Support can be used for Professional Services whereas Product Support cannot.

This transfer cancels Product Support effective the transfer date above; assigning half of the unused portion of Product Support to Product Upgrades and the other half to a Professional Support credit (Retainer Credit).

Your Retainer Credit is $_______________.

From the Transfer Date till the end of your term, Professional Support services, when used, will be deducted from the Retainer Credit, until such time the Retainer Credit is used up, after which you will be invoiced for Professional Support services, Net 30.

By signing below Customer acknowledges the transfer of Product Support to Professional Support and Product Upgrades, and agreement with Aestiva's Professional Support and Product Upgrade agreements.
